CONSENT FOR USE OF IMAGE AND SPECIFIC TERM FOR HOSTS ARRANGING AN EVENT for use in the United States

I understand and agree that by participating in Cozymeal events, photo shoots and video shoots (“Events”), which will take place either at my own location, a third-party location or outdoors, photographs, videos, and audio of me may be taken during such Events, and that photos, videos, and audio of me, my venue or food prepared by me as well as my name, image, likeness and statements may be posted on the Cozymeal website (www.cozymeal.com) (the “Website”), Cozymeal partner websites, social media channels, emails and in other marketing material for promotional purposes. The foregoing sentence applies equally to any photographs, videos, and profile descriptions that I provide to Cozymeal.

I hereby authorize Cozymeal, Inc. (“Cozymeal”), and/or their agents, contractors, service providers or assigns to take photographs and videos, as well as record audio, of me and my venue in connection with Cozymeal Events and the absolute and irrevocable right and unrestricted permission concerning any photographs videos, and audio that are taken of me, my venue and food prepared by me or may be taken of me or in which I may be included with others, to use, reuse, publish, and republish the photographs, videos, and audio in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, editorial, advertising, and trade, without restriction as to alteration; and to use my name in connection with any use if Cozymeal so chooses. I release and discharge Cozymeal from any and all claims and demands that may arise out of or in connection with the use of the photographs, videos, and audio, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also insure to the benefit of the licensees, and assigns of Cozymeal. I understand and agree that, as between Cozymeal and me, Cozymeal shall own all right, title and interest in such photographs, videos, and audio.

I understand and agree that by participating in Events, I am inviting members of the general public who may be unknown to me into my home, venue or place of work. I understand that Cozymeal does not, and has no duty or obligation to, conduct any pre-screening or background checks on any members of the Website. I understand and agree that the Website merely acts as a platform to permit Website members to connect with each other and organize and attend certain social events, and as such, I agree that Cozymeal exercises no control and has no authority over Events, or the actions, events and occurrences that take place therein. Accordingly, I hereby waive, forego, forfeit, and release, to the fullest extent permitted by law, on my own behalf and on behalf of my assignees, heirs, executors, guardians, and legal representatives, any and all claims against Cozymeal based on, related to or arising from my participation in any Event including, but not limited to, claims for personal injury, property damage, economic loss or wrongful death, whether caused by negligence or other acts.

I AM AWARE THAT THERE ARE INHERENT RISKS, HAZARDS AND DANGERS ASSOCIATED WITH AN EVENT AND I VOLUNTARILY ASSUME ALL RISK OF LOSS, DAMAGE OR INJURY TO PERSON OR PROPERTY, INCLUDING BUT NOT LIMITED TO PERSONAL INJURY, SERIOUS BODILY HARM OR DEATH, WHICH MAY ARISE FROM OR IS RELATED TO MY PARTICIPATION IN THE EVENT, WHETHER SUCH RISK IS KNOWN OR UNKNOWN TO ME. I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I UNDERSTAND THAT I CANNOT ORGANIZE THE EVENT VIA THE WEBSITE UNLESS I ENTER INTO THIS AGREEMENT, WHICH I AM DOING VOLUNTARILY AND OF MY OWN FREE WILL.
